LD Resources Foundation ACTION has created checklists, available for download, to assist you in determining whether you, your child or your student may have a learning disability. You are free to download these checklists or just the one that is of interest to you.

A following checklist is just a screening tool; however, there is ongoing research showing that these questions may provide a useful method to identify signs of learning disabilities including Dyslexia.
Feel free to print this report and use it for assessment.

Questions for young children
Problems with language development
Does your child or your student:
1. Have difficulties learned a language?
2. Start speaking later than the norm?
3. Have a limited vocabulary?
4. Mispronounce words or use an incorrect word that sounds similar?
5. Have trouble organizing your thoughts and/or trouble finding the right words?
Problems with reading, writing and/or comprehension
6. Exhibit the following weakness: spelling, trouble remembering sounds and letters?
7. Confuse words that look or sound alike?
8. Struggle to express ideas in writing?
9. Have a problem connecting letters symbols into words; often can’t pronounce a word?
10. Have trouble finding the word needed when writing or in conversation?
11. Have difficulties reading acronyms, symbols?
12. Confuse math symbols and misread numbers reverse them?
Problems with organization, coordination, self-confidence
Does your child or your student:
13. Have issues with social rules skills such as – trouble taking turns, difficulty keeping up a conversation?
14. Have very messy handwriting, can’t stay on the line, struggle to hold a pen or a pencil?
15. Often make mistakes reading aloud; are embarrassed to read aloud?
